Consumption of caffeinated coffee and tea is associated with a decreased glomerular filtration rate 

Chronic Kidney Disease (CKD) is a major public health problem worldwide, affecting 10% of the world’s adult population and more than 20% of those over 60 years of age.

It is a multifactorial disease, growing and costly, which has a notable influence on morbidity, mortality, and quality of life of kidney patients, especially in older people with overweight and obesity.

We refer to CKD to describe the gradual loss of kidney function, when our kidneys fail to filter waste and excess fluid from the blood, building up dangerous levels in our body.

Caffeine and theine

Its high incidence urgently requires effective and efficient population preventive strategies to combat or delay the progression of CKD.

This is the context that has focused the study of the CIBEROBN team led by Jordi Salas and Andrés Díaz from the Rovira I Virgili University (URV) within the framework of the PREDIMED-Plus project.

The scientists explored the possible association between coffee, tea, and caffeine consumption and one-year changes in glomerular filtration rate, a marker of kidney function. To this end, they carried out a study in collaboration with 23 other Spanish research centers and with the participation of 5,851 overweight/obese elderly people with metabolic syndrome.

Dietary Components and Kidney Function

Many dietary components play a major role in preserving kidney function and preventing/slowing the progression of CKD, such as coffee and tea, two caffeine-containing beverages that are widely consumed around the world.

The coffee seed is characterized by its high content of polyphenols (with antioxidant properties), caffeine, vitamins, minerals, and bioactive phytochemicals with potential beneficial effects on health. In this context, the results of epidemiological studies on coffee and tea consumption and renal function have been inconsistent.

The authors found that consumption of caffeinated coffee and tea, but not decaffeinated coffee, was associated with decreased glomerular filtration rate at 1-year follow-up.

After considering possible confounding factors, it was observed that people who drink more than 2 cups of caffeinated coffee per day (approx. 50 ml per cup) showed a greater deterioration of the glomerular filtration rate than occasional coffee consumers (less 1 cup a day) or non-tea drinkers. Likewise, consumption of more than 2 cups of caffeinated coffee per day was associated with a 19% higher risk of having a rapid loss of kidney function.

Potential Side Effects

“Coffee and tea are rich in bioactive compounds with antioxidant and anti-inflammatory properties, with possible beneficial effects on human health,” explains Jordi Salas, one of the main authors of the study and coordinator of the study.

However, it points out that “there are potential side effects of the caffeine contained in these drinks that, even at moderate consumption doses, can influence cardiovascular status through CKD and are a cause for concern for the general population and especially for susceptible or at-risk individuals.

Choose decaf coffee

Based on the study results, consumption of caffeinated beverages may be a concern for the kidney. For this reason, researchers advise elderly people with obesity and high cardiovascular risk to reduce or replace the consumption of caffeinated coffee with decaffeinated coffee to preserve kidney health.

Salas points out that “it is an observational study, so the results must be confirmed by a clinical trial.” If proven, advice on coffee and tea consumption along with caffeine intake should be included in nutritional strategies for kidney disease prevention, particularly in adults at high risk.
